Kettering Health Sports Medicine will be holding two nights of sports physicals for student athletes of Eaton, National Trail, Preble Shawnee, and Twin Valley South. Sports Physicals will be held on Thursday May 5th and Wednesday May 25th from 6-8pm. Student athletes registering for sports physicals must be going into grades 7-12 next year.
COST: $20
Option 1: use coupon code "PAY THERE" when you register and money will be due (cash or check) at the time of your physical. Checks payable to Kettering Health Sports Medicine.
Option 2: Pay online when you register. BE AWARE: no refunds will be available if you pay online, however we can offer you a different night and/or location.

Please update and complete your athlete's Final Forms profiles prior to physicals. Athletes print your “OHSAA PPE for Physicians” from Final Forms and bring it with you to your physical with signatures completed. Registration will be held at the Physical Therapy front desk in the main lobby. So be sure to enter through the physician office doors shown below.
At this time, masks are required in our Kettering Health facilities so we ask that you and your student athlete mask while in the medical center.
WHAT TO BRING WITH YOU:
Please bring your completed OHSAA PPE Physical (For Physicians), printed from Final Forms via the blue icon with all signatures completed.
Payment in form of cash or check ($20 per athlete). Due at the time of physical, unless you paid online. Checks made out to Kettering Health Sports Medicine
Per OHSAA guidelines, physicals completed with Kettering Health Sports Medicine will be kept on file and good for the entire 2022-2023 school year! Proceeds will go to your athlete’s Sports Medicine program supporting supplies for the Athletic Training Room.
These physicals would not be possible without the collaboration of Kettering Health Sports Medicine, Orthopedic Associates of SW Ohio, Preble County Chiropractic, Preble County Medical Center and the Primary Care Physicians within the building.
